Cut these templates into single cards and print on your computer as an envelope one at a time.  After printing this practice template, download the card template below.  You can use the card template to print the practice cards.  First Response will not reimburse you for misprinted cards.  Make sure your template is printing correctly on the practice templates before you print your cards.

                      *** The Instructor is responsible for card print errors. Please ensure that your printer is calibrated to the template & card and feed the card like an envelope.

Instructions for filling out and submitting a roster:

Click on a form name to open it. Please note that you must have the free Adobe Reader software (a download link is below) to open these forms. You may print a blank form to be filled out by hand or fill it out in Adobe Reader and print it as a completed form (except for the instructor signature). Figure 1 below illustrates the print buttons to be used in printing a form (some forms may have only one of these buttons, not both). Whether blank or already filled in, simply click one of these buttons to print the form (you may need to pick the printer you want to use, as with printing anything else from your PC).
